I want to say that the last one is by the lady who died shortly after her hair pieces went all over the web (I can't track down her name right now, will keep looking).
Her creative director is named Matthew "Dada" Williams, and her stylist is named Nicola Formichetti. I think who specifically does what beyond that is held close to the vest. She has a whole team called "Haus of Gaga" that these two are in charge of.
